UnitedHealthcare (Underwritten by Golden Rule) offers two dental insurance plans in Florida and Ohio. The dental can be purchased separately or combined with a Florida health insurance quote. Currently, premiums range from $17.00-$35.00 per a member.

    -A choice of two dental plans that can be used in and out-of-network

    -Only a $50 deductible on Basic (6 month waiting period) or Major Services (12 month waiting period)

    -Annual maximum benefit of $1000 per a covered subscriber

We recommend visiting Dentist Locator at UnitedHealthCare Dentists To check and see if your dentist is in or out of network. If you dentist is out of network the Premier plan would be a better option.

*All basic and major dental work is subject to $50 deductible and six month waiting period for basic work and twelve month waiting period for major work.

There is No waiting period for preventative dental services.

Preventative includes two cleanings and oral evaluations per a calendar year, routine x-rays, fluoride treatment, sealants, and space maintainers.
